Change your Primary Care Provider

You can submit a request to change your Primary Care Provider (PCP) using the request form on this page. Your request will be directed to the Member Services department. Please note that your request must be sent before the 25th of the current month for your newly selected PCP to become effective on the 1st of the following month. If you send in your request after the 25th, your newly selected PCP will NOT be effective on the 1st of the next month, but rather on the 1st of the next month following. You will also be sent a new Member ID card to reflect the PCP change.

Examples:
- If you send in your request on the 15th of July, your new PCP will be effective on 1st August.
- If you send in your request on the 26th of July, your new PCP will be effective on 1st September.

Finding a PCP

Use our online provider directory to search for a PCP. When you have located one, enter the provider's ID in the 'New PCP ID' field in the Change Request Form. Medicaid members, please call customer service at (313) 871-2000.
